Reserve Multimedia Equipment

Complete the form below and select the checkbox next to each piece of equipment you would like to check out, along with the quantity needed. Only two video cameras may be checked out at a time.

Please be sure to familiarize yourself with the multimedia equipment policies before completing this form.

You will be asked to show a valid ID (JagTag, Driver's License, or State ID) when you pick up the equipment you have reserved.
